Pro-Med Plus is a membership program designed to help cover the costs of an ambulance transport. For $39.00 a year, we will bill your insurance company and then Pro-Med Plus covers what your insurance does not. Pro-Med Plus members have no out-of-pocket expense for medically necessary ambulance service.

Treat and release without transportation to a hospital is NOT a covered benefit of this program.

Pro-Med Plus starts June 1 and ends May 31 of each year. To join, print, complete, and sign the attached application. Mail it to Pro-Med with your check or money order. Visa and MasterCard are also accepted. You will receive membership confirmation and membership cards by mail, and a copy of the membership copy.
